A relapse prevention plan helps people in recovery anticipate and manage high-risk situations, cravings, and triggers to avoid returning to substance use or unhealthy behaviors. It involves developing coping skills and building a strong support system. The goal is to replace unhealthy behaviors with healthier habits and increase self-confidence in the ability to maintain long-term recovery.
If you’re recovering from a substance use disorder, it’s important to have a plan written out and shared with others, such as friends, family members, or members of your professional care team.
Work with your loved one or client to complete the Relapse Prevention Plan together. The person completing the plan should keep a copy in an easily accessible location and the family, friend, or provider should also keep a copy of the plan.
